Office supplies - HCFA forms, envelopes, labels, paper and other miscellaneous items The following are additional costs that may apply to you: Postage for the mailing of HCFA forms and patient bills A $15 per month software fee Credit card processing fees There are no start-up, cancellation or hidden fees.
